ATHLETIC DEPARTMENT MISSION AND VISION STATEMENT

WINTER SPORTS REGISTRATION HAS CLOSED!
SPRING REGISTRATION SHOULD OPEN IN MID-FEBRUARY MORE INFORMATION TO COME.
Click on the Online Registration link below under the Parent and Athlete Information
You will need to upload a copy of your Insurance Card, Physical and Health History Form again.
The Regsitration form was changed slightly for the Winter season to make it a little more user Friendly and therefore that information didn't carry over.
If you didn't save a copy of those documents after uploading them, You should be able to get into to your fall registration and be able to view and save them.
Once saved you can upload them to the new location for the Winter Registration.

Online Registration - This is a new registration site. You will need to create a new account.
2025-2026 Extra Curricular and Interscholastic Handbook
PIAA Physical Form - Section 6 Initial Physical Form. Must be dated after May 1st of each year for the upcoming school year.
PIAA Health History Form - Section 5 of PIAA Form. Some physicians will require this information when completing the physical.
PIAA Dr. Recertification Form - Section 8 of PIAA Form. This needs to be completed when there has been an injury or illness since the initial physical and the athlete needs to be cleared by a doctor to return to play.

Online Ticketing Site - You can use this to purchase tickets in advance. Tickets can still be purchased at the gate.
Admission Fees will be charged for the following sports:

Varsity/JV:
Adult - $5.00
Student - $3.00
Jr. High:
Adult - $3.00
Student - $2.00
*KCSD Students get in Free with their Student ID*
Sr. Citizen Gold Card Pass (Free) -
- Must be 62 Years if Age
- Must be Retired
- Must live in Keystone Central School District
Online Application for Pass - Once this is submitted the pass will be processed and mailed to you.
Paper Application for Pass - The completed form along with copy of drivers license can be emailed to athletics@kcsd.k12.pa.us or mailed to KCSD Athletics, 86 Administration Dr, Mill Hall PA 17751. Once received the passes will be processed and mailed to you.

Athletic Employment Applications - Please make sure you are completing the correct application for the position you wish to apply for. (All Volunteers must see the Head Coach to obtain the volunteer application.)

Student Game Worker Requirements: must be a KCSD Student at least 14 years of age. If you are under the age of 18 you must obtain working papers from the main office at the school.
All clearances and volunteer disclosure forms should be sent to the personnel secretary or uploaded to your application.
